Outside Las Vegas
1. Print LAB Rx form and complete.
2. Call Alfredo for starter kit or place case in zip-lock bag and then in cardboard box. Include LAB Rx with case.
3. Address to Logic Dental Design.

- Go on-line to UPS or FEDEX to schedule a pick-up.
- Drop off at an Authorized shipping center or drop box.
4. Call Alfredo at 866.876.3588 to confirm seat date.
